Position Overview:Under the direction of the Supervisor of Prevention Services and in collaboration with colleagues on the treatment team the Social Worker participates in the planning and delivery of evidenced based clinical services and assures fidelity to the particular evidenced based model in terms of service delivery. Position Specifications:
- Master’s degree in Social Work from an accredited university social work program (M.S.W.) or related field, required.
- LMSW, LCSW, LMHC required within one year of hire date.
- Local travel is required; A current Clean & Valid New York State Driver’s License (actively licensed at least one year) and reliable vehicle for transporting clients to community resources, meetings, etc. - with proof of 100/300 liability insurance coverage - is required.oStaff in driving positions must be able to meet the minimum requirements of our agency’s insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire.
- Previous experience working in a human services agency is a plus!
- Proficiency in various computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Sheets, Docs, Meet, etc.), and use secure Internet practices.
- Experience with AWARDS is a plus!
- Ability to function well in a high-paced and, at times, stressful environment.
- Ability to handle sensitive data with strict confidentiality.
- Ability to pass required pre-employment background checks, to include fingerprinting.Duties & Responsibilities include:
- Provide evidenced-based clinical services to children and family as delineated and prescribed in the treatment planning process. The clinical components of these services will include completion of the assessment format, negotiation with clients as to the types of intervention that will be deployed, the time and place parameters of the service delivery, schedules for Service Plan Reviews and projected time frame for discharge from services.
- Provide accurate and thorough documentation of client contacts via the New York State Connections system and with the specific St. Catherine’s AWARDS system.
- Provide specialized therapies including play therapy for children, individual counseling, couple counseling and family counseling as prescribed by the treatment team.
- Follow Albany County and St. Catherine’s requirements for frequency of contacts and for place of contacts as described in the Albany County DCYF and St. Catherine’s contract.
- Clinician will adhere to the CASSP Core Principles, affirming a client’s right to self determination and full involvement with the treatment team in terms of developing service goals and consumer friendly methods for attaining these goals; assuring that services are child centered, family focused, culturally competent and community based.
- Chair Service plan reviews as needed.
- Maintain regular contact with relevant agency staff and other community agencies providing services to families.
- This position will have full and complete access to client Protected Health Information (PHI) in order to carry out the duties and responsibilities for this position.
- Additional position-related duties and responsibilities as assigned by the supervisor/ director.Organizational Relationships:
- Collaborates with staff across all agency programs to ensure quality environments and adherence to regulations.
- Meets weekly with Assistant Director of Clinical Services/ Deputy Executive Director for CQI/ Strategic Planning and have regular communication with the agency staff.
- Ability to use and apply sanctuary values as outlined in The Sanctuary Model®.
