Duties Description POSITION DESCRIPTION:The selected candidate will be a vital team member within the Region 1 Regional Office organizational structure. The SWA2 will typically provide support to one designated functional team within the Regional Office but may provide additional or alternative coverage when Regional Office needs deem this necessary. The functional teams within the Regional Office include: Front Door, Waiver Services, Eligibility, Self-Direction, Housing, Crisis, Property/Development, Fiscal, and Quality Management/Technical Assistance. The selected candidate will work to ensure excellent quality in Regional Office operational standards.Occasional evening and/or weekend work may be required to meet organizational needs. Occasional travel within the 17-county region may be required:PRIMARY DUTIES and RESPONSIBILITIESSpecific responsibilities will include, but are not limited to:1. Serve as a member of the Regional Office providing the full range of services to individuals and families.2. Participate in activities such as screening and/or assessing individuals psychosocial needs, strengths and limitations.3. Assist with reviewing and assessing individual’s need for OPWDD services and supports.4. Communicate with key stakeholders, including staff in various departments in the Regional Office, Central Office, Care Management Agencies and Voluntary Provider Agencies.5. Serve as a liaison between the Regional Office and the individuals/families we serve, along with provider agencies and other referring entities.6. Attend and participate in housing related meetings, telephone conference calls and video conferences.7. Facilitate review and processing of certified housing referrals and other required forms, in addition to other duties as outlined in the CRO ADM.8. Facilitate assessing and sending of certified housing referrals to provider agencies for screening in order to fill certified residential vacancies.9. Provide support across the various departments in the Regional Office as needed.10. Additional duties may be assigned to assist in supporting Regional Office operations.LinkedIn:

Qualifications Promotion: one year of service as a Social Work Assistant 1.Open Competitive:A bachelor's degree in a human services field and two years of social work experience.Substitution:Master's degree in a human services field can substitute for one year of specialized experience.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.
