The Opportunity

FCA is seeking an Intensive Case Manager for our EISEP(Expanded In-Home Services for the Elderly) Program.

The Case Manager provides supportive case management to facilitate connection to services that improve overall Quality of Life. The goal is to connect seniors with socialization to reduce loneliness, technology assistance, address social determinants of health and aging needs. The focus is to break down the barriers to these services and resources for clients 60 years and over, who reside in Nassau County.

Responsibilities

• Provide an initial assessment of the senior adult's needs, develop a plan for services and assist with the implementation of this plan.
• Work with the EISEP case managers to provide more intensive follow up for clients determined to be at risk by initial EISEP assessment screening results or demonstrating an inability to connect with services.
• Provide information and referral to clients as requested.
• Provide advocacy for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves.
• Prepare and maintain client case notes and prepare and keep statistics as required by the standards set by FCA.
• Outreach within the community to provide information and assistance.
• Assume other tasks and responsibilities assigned to support the operations of the program.
