The Foster Care Coordinator role involves implementing trauma-informed, evidence-based strategies for children and youth placed in out-of-home care via the foster system. We focus on those with complex/high or moderate needs who are not enrolled in a home and community-based waiver or receiving services through a Certified Community Behavioral Health Clinic (CCBHC).
This is a field-based position with flexibility to telecommute in Kansas, and serves Saline County and surrounding areas.
Primary Responsibilities:
- Develop, evaluate, and implement personalized care strategies for children and youth within a family-oriented approach, considering their unique needs and promoting the least restrictive level of care.
- Identify, initiate, and coordinate referrals for medical, psychosocial, and behavioral services to meet each member’s needs.
- Act as the single point of contact to manage care plans throughout the continuum of care, ensuring consistency and quality.
- Facilitate communication among all stakeholders to guarantee that vital health-related information is shared, thus enhancing the quality of care provided.
- Advocate for children and families to ensure their needs and choices are central to the decision-making process.
- Provide education on trauma-informed care and specific disorders to members and caregivers to promote understanding and support.
- Collaborate with child welfare and juvenile justice systems to support the members and their families.
- Coordinate additional services such as educational advocacy, behavioral health services, and early childhood intervention, aligning with state regulations.
- Work alongside a master's-level case manager for complex cases to review the assessments and care plans.
- Required qualifications:
- Master's Degree in Social Work, Nursing, Psychology or a closely related field.
- Active unrestricted licensure as a Registered Nurse.
- Minimum of 2 years of experience in a community health setting.
- At least 1 year of experience working with multi-system children.
- Proficient with MS Word, Excel, and Outlook for documentation purposes.
- Willingness to travel within the region to visit Medicaid members at various locations, such as foster homes and community centers.
- Access to reliable transportation and a valid US driver's license is required.
Preferred Qualifications:
- Licensed Social Worker or equivalent clinical degree.
- Experience in managing populations with intricate medical or behavioral needs.
- Familiarity with electronic charting systems.
- Proven ability to arrange community resources effectively.

The expected salary range for this position is $28.61 to $56.06 per hour, based on full-time employment. Pay varies depending on factors like local labor market, education, and experience. UnitedHealth Group adheres to all applicable minimum wage laws.
