Position Title: Foster Care- Care Manager
Location: Remote-KS | 50% travel to member locations
Duration: 3 Months | Possible extension or conversion
Schedule: 8:00 AM – 5:00 PM (Some flexibility based on member needs)
Pay rate: $40-45.00/hr
Key Responsibilities:
- This is a remote position with up to 50% travel meeting with members in their home and providers within Sedgwick and surrounding counties
- Develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.
License Required:
- Licensed Master Social Worker (Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience required)
Education & Experience Requirements:
- Master’s degree in Behavioral Health or Social Work, or Degree from an accredited School of Nursing, with 2–4 years of relevant experience.
- Must haves: Experience working with Foster Care Youth, Child Welfare, Mental Health
- Must have good driving record due to member visits
- Nice to haves: experience in BH, trauma informed care, experience with working with individuals w/ mental illness
- Disqualifiers: short tenure may be questionable but can depend on the reason. Looking for longer tenure. Consistency is key in this role
- Performance indicators: state contract requirements that MUST be met; must complete assessments, documentation, etc. in a timely fashion; consistency in attendance, as well
Job Types: Full-time, Contract
Pay: Up to $45.00 per hour
Expected hours: 40 per week

- Monday to Friday
