About Job
A Social Worker is a part of the interdisciplinary team of clinicians to evaluate patient’s levels of social and emotional functioning
Responsibilities
- Psychosocial assessment of strengths and weaknesses, including coping skills, support systems, and the need for community resources
- Assessment of grief reactions and high-risk factors for complicated bereavement
Qualifications
- Licensed at the LICSW, LCSW level by the Board of Registration in Social Work pursuant to G.L. Chapter 13, Section 80
- Minimum of 1 year experience in Hospice and Home Care
- Computer experience
- Requires ability to work independently, maintain confidentiality
- Excellent communication skills
- Driver’s License
