Position Summary:
We are currently seeking a compassionate and experienced Licensed Master Social Worker (LMSW) for a Post Acute Care setting (LTC/SNF) at a facility affiliated with Providence Health and Services. This role involves a wide range of social work responsibilities focused on assessment, care coordination, discharge planning, and counseling in an inpatient setting.

Requirements:

• Active AK LMSW license
• BLS certification
• Minimum 1 year of recent experience in a Post Acute Care setting (LTC or SNF)
• Experience with biopsychosocial assessments and discharge planning
• Proficiency in any charting system
• Knowledge of community programs, protective services, and counseling techniques
• Travel experience is a plus, but not required

Job Type: Contract

Pay: $55.00 - $58.00 per hour

Expected hours: 40 per week

• 8 hour shift
• Day shift
