Position Overview: We are currently seeking a dedicated Hospice Licensed Social Worker (LBSW) to become an essential member of our interdisciplinary team.

Key Responsibilities: The Hospice Social Worker (BSW) will primarily focus on delivering psychosocial support to patients and their families. This role involves conducting thorough assessments to identify needs, goals, interventions, and services as part of the established plan of care. The Social Worker will use professional training and sound judgment to guide the psychosocial assessment process effectively.

Qualifications:

  • Bachelor's Degree in Social Work from a CSWE accredited institution.
  • Licensure as required by state regulations.
  • A minimum of one year experience as a Social Worker in a healthcare or hospice environment; three to five years preferred.
  • Expertise in dealing with terminally ill patients and their families, along with an understanding of hospice care and the psychosocial aspects of illness, loss, and death.
  • Ability to provide compassionate support during a time of crisis and cope with the emotional stress of frequent loss.
