Job Summary
The Care Coordinator plays a crucial role in supporting the coordination of patient care services. They work closely with healthcare providers to ensure seamless delivery of care to patients.
Duties
- Coordinate and schedule patient home care services.
- Maintain accurate and up-to-date patient records
- appropriately staff patient's cases
- Communicate with healthcare providers, patients, and families
- Provide administrative support
- Utilize medical terminology appropriately
- Collaborate with the agency healthcare team to ensure quality patient care
Experience
- Previous experience of patient care coordination in the licensed home care agency
- scheduling and staffing of patient's cases
- Familiarity with medical terminology is a plus
-computer literacy, knowledge of HHAEX system

Pay: From $25.00 per hour
Schedule:
- Monday to Friday
- Weekends as needed
