Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and efficient care. This position involves managing patient care plans, facilitating communication between healthcare providers, and supporting patients through their healthcare journey. The ideal candidate will have a strong background in medical administrative support and be familiar with medical terminology, particularly in dental and medical office settings.
Responsibilities
- Coordinate patient care by developing and managing individualized care plans.
- Serve as the primary point of contact for patients, addressing inquiries and concerns regarding their care.
- Collaborate with healthcare providers to ensure seamless communication and continuity of care.
- Schedule appointments, follow-ups, and referrals as needed to optimize patient outcomes.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Educate patients about their treatment options, procedures, and preventive measures.
- Assist in the management of office operations to ensure a smooth workflow within the medical or dental office.
Skills
- Proficiency in medical administrative support, with experience in both dental and medical office environments preferred.
- Strong understanding of medical terminology to facilitate effective communication with healthcare professionals and patients.
- Excellent organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Strong interpersonal skills to build rapport with patients and collaborate effectively with team members.
- Ability to work independently as well as part of a team in a fast-paced environment.
Job Type: Full-time
Pay: $35.00 - $45.00 per hour
Expected hours: 8 per week
