The Geriatrics and Extended Care (GEC) Social Work Program Coordinator will have primary administrative and clinical responsibility for assigned GEC programs, which may include: ADHC (Adult Day Health Care), PACE (Programs of All-Inclusive Care for the Elderly), in-home respite, Homemaker/Home Health Aide (H/HHA), and Contract Nursing Home (CNH). The incumbent will perform clinical functions as required by program policy, to include assessment, providing direct clinical services to program enrollees, and communication with family, caregivers, and contracted program staff to address and resolve patient concerns. The incumbent will ensure that enrollees in these programs meet administrative and clinical eligibility requirements and are receiving the appropriate amount of services to meet their needs. The incumbent will provide oversight of community agencies to ensure that Veterans are receiving quality services in accord with program mandates. The incumbent will engage in continuous quality improvement of all assigned programs and will develop and expand programs as necessary to meet the needs of Veterans and caregivers.

Cheyenne VA Health Care System (CVAHCS) provides healthcare to more than 28,000 Veterans and families living in the tri-state area of southeastern Wyoming, northeastern Colorado, and southwestern Nebraska. The catchment comprises over 34,000 square miles, with three highly rural locations.

The Geriatric Extended Care (GEC ) Social Work Coordinator will provide the following program oversight functions: ADHC (Adult Day Health Care), PACE (Programs of All Inclusive Care for the Elderly), in-home respite, and Homemaker/Home Health Aide (H/HHA) programs. This may include stakeholders (e.g. veterans, families, community hospitals, nursing homes, and community agencies)Other duties include, but are not limited to:

  • Screens and assesses Veteran and their caregivers psychosocial needs and barriers in order to facilitate access to appropriate VA and community resources.
  • Ensures these programs are administratively and clinically in compliance with all National and local VA policy and directives.
  • Responsible for assuring that the placement of Veterans into GEC programs meets VA administrative and clinical eligibility requirements.
  • Reporting and investigating of sentinel/adverse events by program staff as necessary.
  • Responsible for tracking, monitoring and coordinating consults and referrals, and is accountable for clinical program effectiveness and modification of service.
  • Develops a data monitoring system to measure the efficacy of the program.
  • Collects, analyzes and reports program data to local management, VISN Lead and the National Program.
  • Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes.
  • Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
  • The social worker will enter all Veteran/family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to insure information on the patient is shared with other VA staff. This information will be reviewed by a supervisor or peer reviewer on a regular basis.
  • Establish and maintain positive working relationships with employees, volunteers, patients/consumers and other stakeholders within the VA and in outside community agencies.

Work Schedule: Full-time, Monday-Friday, 8:00am-4:30pmPay: Competitive salary and regular salary increases

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Basic Requirements:



  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
  • Physical Requirements. See VA Directive and Handbook 5019, Employee Occupational Health Services.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C.
  • 7403(f).

Grade Determinations:

  • GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:

(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.(c) Ability to supervise multidisciplinary staff assigned to the program.(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(e) Ability to provide training, orientation, and guidance within clinical practice.

Physical Requirements:
Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies such as papers, books, or small parts. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others.

This position also requires close visual acuity (near vision correctable at 13" to 16") to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, hearing (aid permitted), and clear distinctive speech are all required.

Experience working with the Geriatric population, experience with program development.
