Job Overview
We are seeking a dedicated and compassionate Hospice Social Worker to join our team. In this role, you will provide essential support and guidance to individuals and families facing various challenges. You will utilize your clinical counseling skills to assess needs, develop treatment plans, and implement therapeutic interventions. The ideal candidate will have experience working with diverse populations, including individuals with autism, and possess strong crisis management abilities.
Responsibilities
- Conduct assessments to identify client needs and develop appropriate treatment plans.
- Provide individual and group counseling sessions to clients, utilizing evidence-based practices such as cognitive behavioral therapy.
- Facilitate group therapy sessions aimed at promoting mental health and well-being.
- Maintain accurate medical documentation in compliance with regulatory standards.
- Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive support for clients.
- Implement crisis management strategies to assist clients in acute situations.
- Educate clients and their families about available resources and coping strategies.
- Stay informed about medical terminology and current best practices in social work.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution is required.
- Valid state licensure as a Social Worker (LCSW or equivalent) preferred.
- Experience in clinical counseling, individual group counseling, and mental health counseling is highly desirable.
- Familiarity with working with individuals on the autism spectrum is a plus.
- Strong skills in crisis management and the ability to handle high-pressure situations effectively.
- Proficient in medical documentation practices and understanding of medical terminology.
- Excellent communication, interpersonal, and organizational skills are essential for success in this role.
