- Performs patient assessments and interviews for patients referred to Social Services to determine discharge needs. Uses tools of observation, interviewing and data history to meet and accommodate the needs of infants, children, adolescents, adults, & geriatric patients. Serves as a patient advocate.
- Confers with physicians, PCC's, nursing staff and other disciplines within the care continuum regarding patient assessment & needs. Participates in the daily Multidisciplinary meetings and family care conferences.
- Identifies problems and gaps in services both in and outside of the hospital setting for patients. Works with the PCC, nursing, patient/family/significant other and medical staff to provide a smooth transition of care.
- Provides information, education and assistance in completing the following documents: Advanced Directives, Guardianships, Incapacitation and Do Not Resuscitate.
- Assists in maintaining department policies and procedures. Actively works with the Ethics Committee and provides Grief Counseling as needed.
- Works with community agencies and projects to identify needed services and/or provides community education.
- Identify ethical dilemmas, such as conflict of interest, and resolve such dilemmas reasonably through appropriate consultation and moral action.
- Bachelors required
- Masters preferred
- One year prior experience in medical social work preferred
- Excellent interpersonal skills; ability to perform public speaking; ability to effectively communicate with patients, physicians, nursing and other hospital staff, patient families, and general public; perceptive of non-verbal communication
- Currently licensed to practice as a Social Worker in the State of Wisconsin; Valid, current Wisconsin drivers license upon hire required
