Summary
SUMMARY: Under the direction of the Supervisor of Transitions of Care, provides psychosocial assessment for patients after collaboration with the Case Management team to support the delivery of appropriate services to meet the patient needs. This position partners with Case Management and multidisciplinary teams to assure a cost and quality effective discharge, always promotes patients� rights and maximum benefits from healthcare services.
EXPERIENCE: Prefer one year in the medical field (i.e., hospital, home health or nursing home).
SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills.
OTHER: Computer skills.
EDUCATION: Required: Masters - Social Work (CSWE approved)
