SUMMARY:

Knowledgeable of resources necessary to assist clients and families in searching for and contacting family members. Assists Wraparound Team in developing a multifaceted service plan to facilitate contact and reconnection with chosen family members and extended support network. Coordinates Family Search and Engagement (FSE) activities with client Social Workers (SW) and Probation Officers (PO). Represents SCCS at Team Decision Making Meetings (TDM) hosted by Children and Family Services and at other community functions when requested.


PROGRAM DESCRIPTION:

The San Bernardino Wrap program is a community-based, family-centered, needs-driven program designed to maintain children with emotional disturbances in the community at the lowest level of care possible. The goal of the program is to reduce the risk of out-of-home placement and recidivism by brining individuals, agencies and the community together as the decision-making team with the central focus being to meet the needs of the child and family. Services assist in supporting the mental health needs of children and their families, allowing safe and appropriate care, and promoting coordination and collaboration in care planning efforts with the Children and Family Team (CFT) and other child-serving agencies.

KNOWLEDGE, SKILLS & ABILITIES:

  • Must be proficient in Microsoft Word, Excel and Microsoft Outlook.
  • Must be able to communicate clearly, both verbally and in writing.
  • Must be able to author and produce their own emails, letters, reports and other documents in a professional manner. That is, documents and correspondence should be relatively free of grammar and spelling errors.”
  • Ability to interact professionally and effectively with all levels of employees within the organization, to include peers, co-workers and supervisors.
  • Must be detail oriented and have the ability to complete documentation in a timely manner.
  • Excellent time management, organizational and follow through skills.
  • Ability to appear to work every day and on time. Must have reliable transportation as travel up to 2 hours between office locations and within the community is required.
  • Ability to follow directions from a supervisor.
  • Ability to understand and follow posted, printed and/or electronic work rules and procedures.
  • Ability to accept constructive criticism in a respectful, open and interactive manner.
  • Experience working with people over the phone.
  • Must have the ability to perform efficiently and effectively with multiple interruptions.

JOB DUTIES & RESPONSIBILITIES:

  • Manage Family Finding database on N: drive.
  • Document all services provided in client’s case record.
  • Discuss Family Finding Services with the referred client/family at Client Family Team (CFT). Obtain legal guardian signature on either the Consent to Participate in Family Finding form or the Decline to Participate in Family Finding form.
  • Meets with the client/family in order to identify potential family members that the client/family would like to reestablish contact with.
  • Coordinates Family Finding process with the client’s Social Worker (SW) or Probation Officer (PO) as well as with the Client Team Facilitator (FAC).
  • When/if family members are contacted, collaborates with the SW/PO and the FAC to process the status of efforts to contact selected family members with the client/family and gives contact information as appropriate.
  • Participates along with the CFT in facilitating a team decision regarding contacting, visiting, or other communication between client/family and found family members as appropriate and approved by the client’s SW/PO.
  • Performs other related duties as required and assigned.
Requirements:

GENERAL REQUIREMENTS: Education/Experience

  • High school diploma or GED. Bachelor’s degree preferred.

Licensure, Registration, Certification

  • Valid CA driver’s license and proof of automobile insurance.
  • Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.
