Position Summary

The Assessment Care Manager works collaboratively as part of a care team to guide prospective families through an assessment process to enable efficient activation of services. This includes assessing consumer and caregiver eligibility for services, developing an initial person centric care plan, and preparing families for ongoing services including orientation to Careforth’s digital communication app. Assessments are carried out within established timelines for expedient service activation.

The pay range for this position is $47,700 - $56,000. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.

What You Will Do
- Manage initial contact with prospective families, educating them on service qualifications and assessing for service eligibility.
- Conduct initial assessments and evaluations, typically onsite in assigned territory; guide families through efficient service activation.
- Assess social determinants of health needs; promote and educate families on available services and resources
- Assign and orient caregivers to the Careforth app, Careforth’s HIPPA-secure digital communication platform, supporting effective usage
- Complete assessment notes within a case management system, ensuring a smooth transition to start of service with ongoing case management.
- Ensure audit readiness and compliance to regulatory standards.
- Perform other related tasks as needed.
- Bachelor’s degree preferred, with at least two years of experience working with elders or adults with disabilities is required
- Ability and flexibility to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
- Experience in case management, care planning and assessment, including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
- Tech savvy with the ability to teach others; proficient in common software and web-based applications (e.g. Case Management Systems, MS Office)
- Ability to work in a fast-paced environment meeting departmental goals
