Summary

We are seeking a dedicated CASAC Counselor to join our team at the East Harlem Council for Human Services, Inc. In this role, you will assess for substance use disorder and provide intervention/treatment and community referrals to vulnerable patients of Boriken Neighborhood Health Center. Providing services within a patient centered medical, psycho-social case management and health education model. Coordinate referrals to services available through the East Harlem Council for Human Services, Inc. and other agencies. Actively participate in performance improvement and peer review process. Serve as an integral member of a multi-disciplinary care team.

Requirements

High School Diploma or GED required. Bachelor’s degree preferred.
Minimum of three (3) years' experience in a substance use disorder, mental health and/or social service setting.
Valid unrestricted OASAS certification as a CASAC.
Bilingual-Spanish required.
Strong leadership, excellent organizational and communication skills
Ability to work with a strong team of professionals in a culturally diverse environment
Ability to multitask and work/function in a fast-paced environment
Highly organized and self-motivated

Pay: $47,000.00 - $48,000.00 per year
