Upper East Side Rehabilitation and Nursing Center is looking to hire a talented and hard-working social worker to join our dedicated team of professionals:

Major responsibilities of a social worker include:

• Facilitating resident admissions and discharge planning
• Counseling residents and family members
• Completing all required documentation in the electronic medical record
• Including MDS, care planning, discharge planning and being an integral member of the interdisciplinary team.

Requirements:

• BS/BA minimum
• Master Level MSW
• LMSW preferred
• Long term care and short term rehabilitation/nursing home experience
• MDS and Short Term Discharge planning experience required
• Team player with excellent customer service skills required
