SYNERGY HomeCare offers private duty, non-medical home care services to elderly and disabled clients in the comfort of their own residence. We work as a team to achieve high standards in-home care.

1. Conduct New Client Care Assessment and Care Plan Development
2. Respond to questions regarding client and caregivers’ schedules
3. Notify clients with basic information on caregivers, service start date, and times caregivers will be providing care
4. Develop and document Client Care Plans
5. Familiarize caregiver with client information, direction, time, and basic introduction of clients' needs and diagnoses
6. Complete Assessment document
7. Document skills and attributes of caregiver needs to ensure strong matching to clients
8. Update the office team on scheduling issues and HR recruitment needs
9. Maintain computer schedules and ensure timely data entry for clients and caregivers
10. Maintain confidentiality of all information pertaining to employees and clients
11. Perform other functions as deemed appropriate by the management team
12. Report directly to Owner and/or Office Manager
13. Knowledge on Private Pay Clients is encouraged.

• Associates Degree or higher
• Home care or Health care experience preferred
• Strong phone skills
• Strong problem-solving skills
• Ability to multi-task and work well under time pressure
• Positive, friendly attitude and ability to work well in a close, team environment
• On-call REQUIRED - rotating
