We are seeking a Social Worker (MSW) to become a valued member of our interdisciplinary team.

The Social Worker (MSW) plays a crucial role in delivering psychosocial support to the patient and family unit. You will conduct assessments to identify needs, set goals, apply interventions, and connect services according to the established care plan. You will utilize your professional training and judgment to oversee the psychosocial assessment process effectively.

Qualifications:

• Master’s Degree in Social Work from a CSWE accredited school
• Licensure as mandated by the state where the hospice operates
• Minimum one year of experience as a Social Worker in healthcare and/or hospice setting; three to five years preferred
• In-depth knowledge of the dynamics surrounding terminal illness, loss, and death
• Ability to apply insights relevant to the unique needs of hospice patients and families
• Empathy and tact when working with individuals during crises
• Resilience to manage the emotional challenges posed by repeated loss
