Overview:

NaphCare is partnering with Everhealth to hire a Medical Discharge Planner just like you to join our team at the Spokane County Jail located in Spokane, WA.

 

 Pay rates vary based on shift, site needs, and experience.  Shifts and hours vary based on coverage needs and can be discussed upon interview.  PRN shifts and hours vary based on coverage needs for FT shifts and will be determined by site leadership and prn employee availability.

Responsibilities:

Responsibilities for Discharge Planner:

 

  • Conduct rounds with physicians to meet with patients to discuss placement orders
  • Assist patients with a specific plan for medication management
  • Include patient in discharge planning
  • Assist patients with the identification of a support network, instructions for future care and follow-up
  • Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
Qualifications:

Qualifications for Discharge Planner:

 

  • Bachelor’s Degree in a human service field
  • Knowledge in the development of discharge plans and coordination with community service providers
  • Valid CPR card
