RN Care Manager
Reports to: Director of Nursing (DON) / Executive Director

This RN position is a full-time shared RN leadership position between the skilled nursing facility and our assisted living communities. Primary duties include:
· Primary contact for hospital and community referrals for all communities, working with both families and hospitals reviewing referrals to determine appropriate admission and community placement. Referral review, tours, work with discharge planners and/or family members to arrange admission.
· Infection Prevention Nurse for skilled nursing facility and assisted living.
· Responsible for assisted living psychotropic medication assessments, resource for change of condition and careplans, responsible for ongoing policy and procedure review of RN delegated services.
· Works with the assisted living community leadership team providing support and guidance for resident care.
· On-Call rotation for SNF every 4 weeks; mostly call support; occasional need to work as a floor nurse.

St. Joseph Residence is a not-for-profit corporation founded by the Religious Hospitallers of Saint Joseph, owned and operated by Catholic Health International. We are a continuing care campus offering an array of services and settings including independent apartments, CBRF, RCAC, and a skilled nursing facility. Scheduled Hour Times:
Office hours are 8 a.m. to 4:30 p.m. Monday through Friday. This is primarily a day shift position however this position is required to maintain a work schedule that ensures the presence nursing leadership on periodic weekends or evenings, nights and holidays. The expectation is that the RN Care Manager work 40 – 45 hours per week and to participate in a call rotation one week per month (may require occasional presence in the building during call week) among nursing leadership to ensure nursing department support as needed.
Position Description:
This position assists our experienced Director of Nursing, in our skilled nursing facility (SNF), in providing daily clinical oversight and support of nursing staff promoting professional standard nursing practice, shared decision-making, interdisciplinary teamwork and collaboration resulting in positive resident and staff outcomes. This is a fast-paced position that requires commitment, proficient organizational skills, and excellent employee relation skills. This position requires daily face-to-face interaction with staff, residents, and family members, problem solving, following up on resident care, updating care plans, and educating and working with staff on problem solving and skill development.
Primary Responsibilities Include:
- Primary referrals contact for all communities to review and work with families, discharge planners, and providers to assess for admission appropriateness, communicate with discharge planners, and arrange admission and/or transfer to another community.
- Liaison between residents, hospitals, and referring providers reviewing admission referrals for the SNF and ALF.
- Assist with order entry for assisted living admissions and work with LPN and RSD on ISP development.
- Responsible for infection prevention and control program for all communities.
- Review ALF medication orders as needed.
- Participate in resident assistant medication delegation.
- Daily monitoring and review of resident admissions, incidents, and change of condition for process completion, appropriate treatment and/or interventions, updating of care plans, and working with community leadership to provide staff education on changes of condition and interventions.
- Complete psychotropic medication review and TD assessments for assisted living residents.
- Review of current assisted living nursing practices and policy review and implementation.
- Participate in QAPI (Quality Assurance Performance Improvement). Complete monthly quality assurance review/reports and report on clinical quality measures related to Resident events, incidents, or practice deficiencies
- On call rotation every fourth week for the skilled nursing facility / assisted living– call is typically via telephone assistance for clinical guidance, however there are days the RN on-call may need to work as a floor nurse at the skilled nursing facility. Our organization strives to maintain leadership nurses work in their leadership roles not routinely working the floor however on occasion it may be necessary.
Job Requirements
- Must be a graduate of an accredited School of Nursing
- Must possess a current, unencumbered license to practice as a RN in the State of WI
- Must possess current CPR Certificate or ability to obtain certification within 30 days of hire
- Must project a positive image of the facility by displaying a high degree of professionalism, demonstrating concern and courtesy in interactions with residents, families, medical staff, employees, volunteers and outside agencies affiliated with the facility
- Must demonstrate the ability to function independently, have personal integrity, flexibility, and the ability to work effectively with residents, personnel and support agencies, and to make independent decisions when necessary
- Must be able to communicate effectively and clearly with the Director of Nursing, Executive Director, coworkers, residents, families, and providers on an ongoing basis, utilizing tact, clarity, discretion and fairness
Preferred Experience / Knowledge
- Experience in nursing specialty of Geriatrics or Long-Term Care
- Familiarity with State and Federal Regulations of SNF and CBRF, or willingness to pursue further training in these areas.
