Hometown Health Center (HHC) is seeking a compassionate and proactive RN Care Coordinator to join our team. This role is essential to promoting continuity of care, improving outcomes for high-risk and underserved patients and supporting a culture of wellness and prevention. As RN Care Coordinator, you’ll serve as a bridge between providers, hospitals and patients—making sure no one falls through the cracks.

Since 2003, HHC has been a cornerstone of care in Central Maine. As a Federally Qualified Health Center (FQHC), we serve more than 8,200 patients annually through over 32,000 medical, dental and behavioral health visits. With locations in Dexter, Newport and soon Palmyra, we’re deeply rooted in Central Maine’s rural communities—offering care where it's needed most. Our new state-of-the-art facility opening in Palmyra will include a wellness center and onsite daycare.

About the Opportunity
This role is ideal for an experienced RN who thrives in a fast-paced, team-oriented environment and is passionate about closing care gaps, supporting patients with complex needs and improving population health metrics. You'll coordinate transitions of care, identify high-risk individuals, promote preventive health measures and contribute to quality improvement initiatives that align with our Patient-Centered Medical Home (PCMH) model.

What You’ll Do
  • Act as a patient advocate to ensure smooth transitions between care settings
  • Participate in daily care team huddles and support pre-visit planning
  • Track and manage patients at high risk for hospital readmission or underutilization of care
  • Provide education on disease management and prevention (including group visits)
  • Collaborate with the population health medical assistant to gather data and support outcomes reporting
  • Monitor and support behavior modification and patient goal-setting
  • Ensure proper documentation and adherence to clinical protocols in the EMR
  • Participate in quality initiatives and support PCMH recognition and improvement efforts
  • Support improvements in policies and workflows for care coordination and clinical staff

What You’ll Need
  • Active RN license required
  • At least 3 years of professional healthcare experience; care coordination experience preferred
  • Experience using an electronic medical record (EMR) system
  • Strong organizational skills and ability to prioritize in a dynamic environment
  • Excellent written and verbal communication skills
  • Problem-solving mindset and ability to work independently
  • Comfortable working across diverse populations and with multidisciplinary teams
  • Solid knowledge of medications, terminology and common lab tests
