Your Role

• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients progress and work with the physicians to ensure quality patient care.

Qualifications

• Active CMA Certification or Registration (nationally recognized)
• Working hours will depend on your time zone - is you live in MST then you will be working PST hours, if CST then you will be working EST hours.
• You must be bilingual in English and Spanish (FLUENT)
• Experience in geriatrics or primary care is required; in-office experience is a must for this role.
• Ability to sit and talk on the phone for long periods of time
• System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
• Must live and work full time in the US - you can work from anywhere within the US but not outside.

The Pay

• The CMA pay starts at $19.35 per hour (40hrs/week)
• The pay structure is a base pay plus a bonus per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
