Must-Have: Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills, knowledge of third-party payment systems, Medicare/Medicaid programs.

Description:
Responsible for coordinating and implementing post-discharge plans.
Assists with advocacy and referrals to community resources.
Obtains, reviews, and analyzes information related to discharge planning.
Assesses and reassesses patient clinical and psychosocial status.
Identifies community resources and facilitates referrals to agencies or programs as needed.
Educates patients and/or families on available community resources.
Facilitates discharge planning with patients, families, and the treatment team.
Documents actions taken in progress notes and assessment forms from initial visit through discharge.
Demonstrates professionalism in actions and job performance.
Understands the needs of different age groups and implements tailored discharge plans.
Shows sensitivity towards diverse ethnic, cultural, and disabling human diversity.
Conforms to standards of patient and family confidentiality.
Assesses patients' physical, psychosocial, cultural, and spiritual needs.
Ensures appropriate arrangements for post-hospital care before discharge.
Provides support to patients and families coping with medical changes.
Confirms treatment goals and plans of care with the treatment team.
Communicates treatment goals or best practices to the treatment team.
Uses electronic referral systems for discharge planning.
Employs supportive crisis intervention techniques.
Collaborates with managers regarding difficult practice issues.
Adheres to state and federal regulations on discharge.
Implements discharge plans in accordance with physician directions and caregiver agreement.
Assesses patient/family learning styles and teaches accordingly.
Collaborates with the interdisciplinary team to develop individualized plans of care.
Maintains updated referral resource lists.
Assesses, coordinates, and evaluates discharge readiness.
Participates in family conferences and interdisciplinary team meetings as needed.
Completes daily discharge planning verbal rounds.
Initiates discharge planning on day one of referral.
Works with third-party payors to meet discharge planning needs.
Makes appropriate outside agency referrals.
Follows through with all aspects of discharge planning across the continuum of care.
Provides supervision to department medical social workers pursuing advanced licensure.
Performs evaluations and assessments related to biopsychosocial aspects.
Maintains current knowledge base of community services through continuing education.
Demonstrates ability to multi-task, set priorities, and maintain organization.
Possesses computer skills.
Floating may be required to various locations within a specified distance.
Float assignments may include duties outside of original job requirements.
