The Music Therapist provides an initial assessment for referred hospice patients, determines therapeutic goals, and works with individuals, family members, and staff to meet these goals using a systematic application of music interventions in the patient's homes or alternate community settings.


Responsibilities:

  • Assess and provide individualized music therapy consultants and services to the patient and/or family

  • Develop individualized music therapy plan of care.

  • Uses creative therapeutic activities for the purpose of improving or maintaining mental and physical function and quality of life.

  • Implement the music therapy interventions in accordance with the interdisciplinary plan of care.

  • Direct and implement legacy projects for patients/families as identified in the plan of care.

  • Direct other care team members in the use of music to achieve individualized care plan goals.

  • Document all services provided to hospice patients and families in the clinical record.

  • Terminate therapy services when appropriate.

  • Support the business development activities of the organization.

  • Provide in-services to hospitals, nursing homes, and other facilities as requested and determined appropriate.

  • Support bereavement activities as needed and appropriate.

  • Provide bereavement counseling to family members as identified.

  • Participate and/or facilitate Hospice memorial services.

  • Facilitate grief support groups as identified by the IDG.

  • Serve as the counselor at IDG meetings, participates in on call rotation, and assists with sign ups and consents for newly referred patients if needed.

  • Educate the hospice staff about the role of music therapy and the use of music therapy intervention with Hospice patients and staff.


Requirements

Bachelor's Degree in Music with a concentration in Music Therapy and must meet educational requirements to achieve certification from the Certification Board of Music Therapist. Bachelor's Degree in Music Therapy or Master's Degree in Music Therapy Preferred. Previous experience with terminally ill patients is highly desirable.
