Job Title: Home Health Community Liaison
Reports To: Administrator
FLSA Status: Non-Exempt / Full-Time

Position Summary

The Home Health Community Liaison serves as the outward-facing representative of the agency. The role’s primary purpose is to build and nurture relationships with referral sources—physicians, hospital discharge planners, skilled-nursing and assisted-living facilities, payers, and community organizations—to generate appropriate home-health referrals and ensure patients receive timely, coordinated, high-quality care at home.

Key Responsibilities

Business Development

  • Identify, prioritize, and regularly visit referral sources within the assigned territory.
  • Achieve or exceed monthly targets for referrals and admissions by executing a structured sales plan and recording activities in the CRM system.

Relationship Management

  • Establish trust through consistent communication, rapid follow-up, and periodic outcome updates to referral partners.
  • Plan and deliver in-service presentations, continuing-education events, community health fairs, and workshops that highlight agency services.

Clinical Knowledge & Education

  • Maintain a solid understanding of Medicare, Medi-Cal, commercial, and VA eligibility requirements for home health.
  • Accurately explain the agency’s clinical capabilities—including skilled nursing, physical, occupational and speech therapy, medical social work, home-health aide services, and specialty chronic-care programs—to professionals and the public.

Intake Collaboration

  • Collect complete referral documentation, verify benefits, and coordinate with the intake team to admit patients within required time frames.

Market Intelligence

  • Monitor competitor activity, regulatory updates, and payer trends; share actionable insights with leadership to inform strategic decisions.

Documentation & Reporting

  • Maintain timely, accurate contact logs, pipeline reports, and key-performance indicators; present weekly progress summaries to the leadership team.

Compliance & Ethics

  • Follow all federal and state regulations, including Stark Law, Anti-Kickback Statute, HIPAA, and CMS Conditions of Participation.
  • Immediately report suspected compliance issues or unethical practices.

Required Qualifications

  • Skills:
  • Working knowledge of Medicare home-health regulations and basic medical terminology.
  • Strong consultative selling, presentation, and negotiation abilities.
  • Proficiency with CRM platforms and Microsoft Office applications.
  • Excellent verbal and written communication; comfortable speaking to groups.
  • Ability to connect with diverse clinical and non-clinical audiences.
  • Licensure & Credentials: Valid driver’s license, current auto insurance

Physical & Environmental Requirements

  • Up to 75 percent local travel.
  • Ability to lift and carry marketing materials weighing up to 25 pounds.
  • Standard office conditions for administrative tasks and adherence to infection-control protocols when entering healthcare facilities.

Performance Metrics

  • Monthly admissions by payer mix and clinical program.
  • Referral-to-admission conversion rate.
  • Accuracy and timeliness of CRM entries.
  • Satisfaction levels of clients and referral sources.
  • Results of compliance audits (goal: zero violations).
