Lead Intake/Assessor - LADC Role

In this role, your primary focus will be conducting intake assessments and evaluations for individuals seeking recovery services. You will ensure each client receives a thorough, compassionate assessment to guide their personalized treatment plan. Intake and assessments are the core of this role, providing a critical foundation for individualized care and long-term recovery outcomes.

Key Responsibilities:

• Lead client intake by conducting comprehensive assessments, focusing on substance use and mental health needs
• Develop individualized treatment plans based on assessment findings
• Collaborate with clients to establish treatment goals and milestones
• Communicate assessment results and treatment options to clients and their families, ensuring clarity and understanding
• Oversee the intake process to ensure timely, efficient service and a seamless experience for all clients
• Support individual and group counseling sessions, as needed, based on client treatment plans
• Supervise and collaborate with Certified Peer Recovery Specialists (RECOVERY Coaches) to provide holistic care

Qualifications:

• Active LADC in Minnesota with a minimum of 3 years of experience
• Proven experience in substance use counseling and conducting assessments
• Ability to manage client intake processes, ensuring a smooth and compassionate onboarding experience
• Strong ability to communicate with diverse client populations
• Comfortable using EMR systems for documentation and assessment tracking
• Compassionate, approachable, and dedicated to empowering individuals on their recovery journey
• Able to pass a DHS licensed background study
