Do you have what it takes to be a Care Manager:

  •  Current RN Licensure in the state of Ohio 
  • BA in nursing preferred/ Associates required 
  • 2 to 3 years clinical practice experience 
  • Well-rounded clinical experiences including work in outpatient settings  
  • Experience with data analysis and health care operations 

 

A day in the life of a HSO Care Manager:

  • Assists patients through the healthcare system by acting as a patient advocate and  navigator. 
  • Provides patient education 
  • Provides team based care through Chronic Care Management services 
  • Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits.  
  • Educates patient and family on ways to reduce ER visits in the future 
  • Participates in Patient-Centered Medical Home team meetings and quality improvement initiatives. 
  • Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
