Shift Hours: Monday- Friday 40 hours weekly

Discharge Planner Responsibilities

Develops/Manages processes and Systems- Assists with contracts in the negotiation of physician managed care contracts, analytics, and financial modeling
Upholds standards/Expectations of HSS Staff-coordinates enrollment in managed care and plans
Upholds standards/Expectations of HSS staff- resolves payor issues, follows up and resolving payment issues with insurance payors
Maintain and create tracking logs for insurance payor concerns, and accountability for closing out items
