Position Summary: The Palliative Care Coordinator is responsible for coordinating patient-centered care to individuals with serious or terminal illnesses and collaborating with healthcare teams to optimize patients’ quality of life through comprehensive palliative care management.

Essential Duties:

  • Completes workflow tasks and assignments in the Electronic Medical Record (EMR) system.
  • Order labs, durable medical equipment (DME), and imaging; coordinate results with Nurse Practitioners (NPs), enter telephone assessments into the EMR.
  • Assist patients and families with risk assessment, screening and education, working collaboratively with appropriate resources.
  • Manage medication refill requests and reconciliations in patient chart.
  • Facilitate faxing documentation to patients’ primary care providers (PCPs) and/or facilities.
  • Accurately and timely document patient care activities in medical records. Assist patients and families with risk assessment, screening and education by working collaboratively with appropriate resources.
  • Ensure timely access to resources such as home healthcare, medical equipment, and community services.
  • Plan for continuing care including community outreach, home care, and hospice services as needed.
  • Serve as a resource for community education and outreach initiatives. Triage patient phone calls, ensuring urgent calls are promptly addressed by the provider.
  • Collaborate with interdisciplinary teams to develop and manage patient care plans. Conduct clinical level of care assessments based on discussions, medical records, and relevant clinical data.
  • Maintains regular, consistent attendance and is flexible to meet the needs of the department.
  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abide by all regulations, policies, procedures, and standards.
  • Performs other functions as assigned and performs other duties as required.

QUALIFICATIONS:

  • Graduate of an accredited Licensed Vocational Nurse or Licensed Practical Nurse program.
  • Minimum of 2-3 years of healthcare experience.
  • Proficient in Excel, PowerPoint, and Word
  • Experience working with Electronic Medical Record (EMR) systems.
  • Strong analytical and problem-solving skills.
  • Solid understanding of scheduling efficiency and patient flow in healthcare setting.
  • Knowledge of medical billing, coding, and insurance procedures.
  • Excellent verbal and written communication skills.
  • Ability to work effectively under pressure while managing time and tasks efficiently.
  • Collaboratively work within the department and across teams, fostering a proactive team-oriented environment.
