Responsibilities for a social worker
- Assists residents with emotional adjustment to facility.
- As directed, interview new residents and records, social history, and assessments.
- Completes the social services portion of the MDS, Care planning, baseline, etc.
- Refers to psychological services as needed.
- Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
- Writes summaries and progress notes as directed.
- Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
- Assists with discharge planning. Follows up after discharge.
- Conducts required Cut Letters
- Writes discharge summaries as assigned.
- Develops a professional relationship with each resident and family.
- Educates residents/ representatives on Advanced Directives.
- Adheres to all Social Services Regulations.
Qualifications for a social worker
- Minimum Bachelor's Degree in Social Work from an accredited College.
- Must obtain a CSW and/or LSW.
- One to Three years of professional experience and/or training; or equivalent combination of education and experience.
