Schedule: Monday-Friday 8a-5p

Your Responsibilities:

  • Conduct thorough psychosocial assessments of patients and their families to determine needs, ensuring they are integrated into the care plan.
  • Provide exceptional service to patients, families, and colleagues, supporting team cohesion and collaborative care.
  • Offer counseling for coping with stress and other challenges faced by patients and families.
  • Document all patient services accurately and promptly to reflect the care plan.
  • Assist patients and families with applications for Supplemental Security Income, Medicaid, and other resources, connecting them with community services to meet their needs.
  • Act as a bridge between patients, families, and community resources to enhance support.

Qualifications:

  • Master's degree in Social Work preferred; Bachelor's degree in Social Work, Psychology, Sociology, or a related field acceptable.
  • Current Social Work Licensure in Ohio (LSW, LISW, or LPSW-S).
  • At least one year of experience in a healthcare or hospice setting.
  • Valid driver's license and proof of insurance.
