Job Title:- Licensed social worker
Job Location:- Washington, DC
Job Type:- Contract/Temp.
Duration:- 13 weeks
Shift/Hours:- Mon-Fri; 8 hours ((08:00 - 16:30); Weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed

• DC RN Licensure - ** WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION* (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW
• BLS

Job Requirement: -

• 2 years of experience: discharge planning to include: admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience; prior charge experience preferred
• Completion of at least 1 travel assignment
• *Regulatory Skills required: CMS, HIPPA, JCC; workers compensation preferred*
