FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a compassionate individual to serve as its Grief Care Coordinator. Our Grief Care Coordinators provide individual and group grief care by assisting families, caregivers, children and adolescents in addressing the anticipatory grief or bereavement needs of hospice family members for a Hospice death and community family members through a non-hospice death. FAIRHOPE looks to its Grief Care Coordinators to walk alongside individuals and families through their grief journey.

Responsibilities:
- Provide child, teen, and adult grief care individually and/or in groups via in-person or telehealth meetings at the Anita M. Turner Center, area schools/facilities, homes of hospice families, or at FAIRHOPE's Pickering House to ensure timely and appropriate grief care is provided. Participate in grief care events as scheduled or as needed.
- Work in conjunction with FAIRHOPE RN Case Managers, Pickering House staff and Social Workers. Seek guidance from other members of the interdisciplinary team as needed.
- Responsible for accurate and timely documentation of grief care on assessments, plans of care, and contact notes. Assist in maintaining the database for the grief care caseload.
- Collaborate with FAIRHOPE Pickering House staff regarding family members who are having trouble communicating or processing their grief reactions related to a patient's dying.
- Assist with providing grief care information to callers from the community.
- Provide grief care to FAIRHOPE personnel coping with work related grief through one-to-one support and referral to community resources.
- Participate in Hospice activities such as in-service education, Hospice personnel meetings, and relevant committees.
- Attend and actively participate in interdisciplinary meetings.
- Remain HIPAA compliant at all times.
Qualifications:
- Bachelor of Science or Bachelor of Arts in the fields of education, communication, or social service practices required.
- Experience meeting with individuals, families, and/or caregivers to address anticipatory grief or bereavement needs related to dying and death required.
- Understanding of grief response and experience with supporting individuals and/or in groups regarding hospice death or non-hospice community death.
- Knowledge of and commitment to the Hospice philosophy.
- Ability to manage time efficiently. Demonstrate great communication and social interaction skills, as well as a compassionate individual.
- Computer skills required.
- Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to FAIRHOPE and/or FAIRHOPE's insurer.
- Physical Setting: Various healthcare settings. Some driving is required.
- Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
