Join Our Team as a Family Success Coach

We are seeking a driven and compassionate non-clinical Family Success Coach to join our dynamic team. As a Family Success Coach, you will play a pivotal role in empowering families through strength-based screenings and assessments, individualized case planning, and comprehensive care coordination. Your expertise will help families achieve their goals and strengthen protective factors.

The Family Success Coach delivers holistic care coordination, fostering engagement, and leveraging wraparound services. This position offers an opportunity to make a meaningful impact on the lives of children, youth, and their families while contributing to a supportive, strengths-based organizational culture.

Key Responsibilities

  • Conduct strength-based assessments to identify family needs and develop individualized case plans.
  • Provide referrals and linkages to services as part of a comprehensive care coordination model.
  • Facilitate one-on-one and small group coaching sessions to support family goals using motivational interviewing techniques.
  • Collaborate with therapists, community providers, and wraparound teams to ensure seamless service coordination.
  • Conduct monthly in-person goal check-ins and coaching sessions with families.
  • Support family engagement and retention through consistent communication, progress reviews, and celebrating milestones.
  • Strengthen protective factors by empowering families to build resilience, stability, and sustainable support systems.
  • Use strength-based and wraparound service principles to co-create family-centered plans tailored to each family's unique needs that reflect and respect their values and traditions.
  • Maintain accurate documentation of assessments, case plans, and progress notes.
  • Participation and implementation of continuous training and professional development to improve family services.
  • Ability to conduct home, school, and community visits
  • Attend required training and team meetings

Ideal Candidate:

  • Bachelor's degree in Social Work, Psychology, or related field; Master's degree preferred
  • Child and Adolescent Needs and Strengths (CANS) certification is required within 20 days of employment
  • Minimum of 2+ years of experience in related social services, managing caseload, and providing prevention and early intervention services directly to families with children/youth.
  • Knowledge of child, youth, and family resources and services in Miami-Dade County
  • Strong communication and interpersonal skills
  • Bilingual in English and Spanish (must be able to communicate fluently in both English and Spanish)

  • Combination of in-office work and home visits tailored to meet family needs
  • Ability to work some evenings to meet families' availability after 5 pm and the occasional Saturday for Community Events

Compensation:

  • Competitive salary negotiable based on experience and qualifications

Background Requirements:

  • State and Federal Background screening is required
  • E-Verify participation required
