Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will have experience working with individuals with specialized advanced dementia and possess strong leadership skills. This role is essential in ensuring that clients receive the highest quality of care and support in their home and community settings. The Care Coordinator will execute care plans as written, facilitate communication among team members, perform medication audits, and advocate for the needs of clients in long-term care environments. Will be supervisor of 12 or more employees.

Duties

• Assist in developing, implementing, and monitoring individualized care plans for clients with specialized advanced dementia.
• Collaborate with healthcare professionals, families, and community resources to ensure comprehensive care.
• Provide guidance and support to staff in nursing.
• Conduct regular assessments to evaluate client progress and adjust care plans as necessary.
• Lead team meetings to discuss client needs, operational management, and service delivery improvements.
• Advocate for clients’ rights and ensure compliance with regulatory standards in long-term care.
• Maintain accurate documentation of client interactions, progress notes, and service delivery.

Requirements

• Experience working with individuals with specialized advanced dementia is preferred.
• Background in social work or nursing home operations management is a plus.
• Strong leadership skills with the ability to manage a diverse team effectively.
• Excellent communication skills to facilitate collaboration among various stakeholders.
• Knowledge of long-term care regulations.
• A compassionate approach to caregiving, demonstrating empathy and understanding towards clients’ needs.
• Ability to work independently as well as part of a multidisciplinary team.
• Must be certified as a Medication technician with a minimum educational requirement of a high school diploma or GED.
• 3 years of experience as a memory care director.

Job Type: Full-time

Pay: $18.00 - $25.00 per hour
