A bachelors degree in SW or a Human Services field and one year of supervised SW experience in a health care setting.

Effective interpersonal and communication skills

Genuine caring for and interest in elderly and handicapped people

Develops a social history and care plan for each resident.

Coordinates discharge planning with staff, Residents, and families.

Provides individual counseling for Residents at times of crisis or particular need.

Participates in interdisciplinary care conferences.
