Job Title: Licensed Master Social Work or Licensed Graduate Professional Counselor

Job Description:

We are seeking a compassionate and dedicated LMSW or LGPC . You will play a vital role in providing comprehensive social work services to individuals, families, and communities. Your expertise and commitment will be instrumental in assessing, addressing, and managing the social and emotional needs of our diverse client population.

Responsibilities:

- Conduct thorough psychosocial assessments to evaluate clients' strengths, challenges, and support systems.

- Develop and implement individualized care plans in collaboration with clients, their families, and other members of the interdisciplinary team.

- Provide counseling, therapy, and crisis intervention services to clients experiencing mental health challenges, trauma, substance abuse, or other social difficulties.

- Offer guidance and support to clients in navigating social service systems, including assistance with accessing housing, healthcare, financial resources, and other community-based programs.

- Advocate for clients' rights and collaborate with community organizations, government agencies, and healthcare professionals to ensure appropriate services and resources are available.

- Maintain accurate and up-to-date electronic records of client assessments, progress notes, and treatment plans, ensuring compliance with legal and ethical standards.

- Collaborate with other social workers, psychologists, psychiatrists, nurses, and healthcare providers to ensure coordinated care and optimal client outcomes.

- Stay informed about current research, best practices, and legal regulations in the field of social work through ongoing professional development activities.
