Overview
We are seeking a dedicated and compassionate Care Managers to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the highest quality of medical services tailored to their unique needs. This position requires a strong background inthe care of elderly or disabled persons.
Responsibilities
- Coordinate and manage patient care plans in collaboration with healthcare providers, patients, and families.
- Conduct comprehensive assessments of patients' medical histories and current health status.
- Conducting on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse/csw per policy) for Level I, and monthly for Level II
- Monitor patient progress and adjust care plans as necessary based on evolving health conditions.
- Educate patients and families about treatment options, medical terminology, and care processes.
- Advocate for patients’ needs within the healthcare system to enhance their experience and outcomes.
- Stay informed about best practices in infectious disease care and emergency medicine to provide up-to-date guidance.
- Document all interactions and updates in compliance with healthcare regulations.
- Travel to member residences/meetings; will require transportation to get to various work sites
Experience
- Clinical experience in the care of elderly or disabled persons
- Familiarity with managed care systems and protocols is beneficial.
- Proficiency in medical terminology to effectively communicate with healthcare professionals and patients alike.
- Ability to work collaboratively within multidisciplinary teams while demonstrating strong interpersonal skills.
- Microsoft Office
Qualifications
- Bachelor's Degree (Mandatory)
- Social Worker License (Preferred)
- (2) years experience clinical/working experience in the care of elderly or disabled persons
- Current CPR
- Must be able to pass a background check
- Must have a Valid Drivers license and Transportation

Pay: $56,771.00 - $59,912.00 per year
