The Health Care Home Nurse Care Manager serves assigned clients as outlined by the client’s treatment plan and indicated level of care to ensure the integrated health needs of those assigned to the caseload are effectively and efficiently served.

Duties and Responsibilities

Educate/train staff and clients pertaining to topics related to medical diseases, treatments, and medications
Develop wellness and prevention initiatives
Facilitates health education groups
Participates in the initial treatment plan development for all Health Care Home enrollees
Consult with Care Coordinators about identified health conditions/concerns
Assist in contacting medical providers and hospitals for admission/discharge
Track required assessments and screenings
Assist in implementing DMH Net health technology programs and initiatives
Monitor HIT tools and reports for treatment and medication alerts and hospital admission/discharges
Monitor and report performance measures and outcomes
Complete all necessary documentation in a timely manner
Complete other duties as required by the position and/or as assigned by the immediate supervisor or the Chief Executive Officer

Qualifications

Education - Minimus of an Associate Degree
Skills/Abilities - Effective oral and written communication
Professional Certification - Licensed or license eligible Nurse in the State of Missouri

Working Conditions

General Conditions- Work is generally performed in office type settings where answering phone and using computers and other technology should be expected.

Special conditions -Meetings with clients outside the office may require ability to tolerate prolonged exposure to smoke and pets.

Physical Requirements

General Conditions- Job duties may require sitting or standing for long periods of time. Travel, with some trips requiring several hours in a vehicle, should be expected. Minor lifting and moving may be expected requiring ability to lift up to 20 pounds.

Special conditions -Meetings with clients outside the office may require ability to enter and access homes of all configurations.
