MDS Care Plan Coordinator RN Job Qualifications:
- RN in WV or in compact state
- Prefer at least 1 year experience as a RN
- Prefer MDS experience
MDS Care Plan Coordinator RN Job Duties:
- Coordinate and assist with completion and submission of accurate and timely interdisciplinary MDS Assessments, and Care Plans
- Follow state and federal guidelines
- Follow facility policies and procedures
- Other duties as assigned
