MDS Care Plan Coordinator RN Job Qualifications:

• RN in WV or in compact state
• Prefer at least 1 year experience as a RN
• Prefer MDS experience

MDS Care Plan Coordinator RN Job Duties:

• Coordinate and assist with completion and submission of accurate and timely interdisciplinary MDS Assessments, and Care Plans
• Follow state and federal guidelines
• Follow facility policies and procedures
• Other duties as assigned
