RN Case Manager- ED

About the Job:

The Case Manager RN will awork primarily in the Emergency Department, and collaborate with other members of the health care team to identify appropriate utilization of resources and a safe and effective transition plan ensuring clinical and social determinants of health are met. With the patient, family/caregiver, and health care team, create a transition plan appropriate to the patient’s needs and resources including community providers to ensure effective communication and collaboration with a successful transition plan.

Schedule:

You will work 36 hours per week, (3 12-hour shifts), and every other Saturday.

What You’ll Do:

• Assesses and identifies patients’ needs during the discharge planning process
• Provide resources and community referrals for continuity of care
• Care coordination with families, providers, and clinical team
• Follows Medicare conditions when needed
• Utilization Reviews

Who You Are:

• Bachelor's degree in a healthcare-related field or equivalent experience
  • Current BSN or commitment to achieving the degree in a specified amount of time
• 3 – 5 years of case management-related health care experience.
• CCM or ACM preferred
• Excellent clinical documentation, and successful usage of EMR systems preferred
