General Description:
The Social Worker is responsible for identifying and supporting the social and emotional needs of the children and adolescents at Cedarcrest, as well as their families. This position involves providing comprehensive social support services, facilitating communication between families and staff, and contributing to a collaborative team environment focused on the well-being of our residents.
Primary Responsibilities
Support Transitions and Admissions
As a Social Worker, facilitate smooth admissions and discharges for children and families.
Provide tours to prospective families and providers, ensuring they are informed about Cedarcrest’s services.
Meet with families of potential residential/day students, helping them understand the process and available support.
Make home and hospital visits as needed to assess and support families.
Assist families with completing necessary admission paperwork, Medicaid, and Social Security applications.
Notify staff of upcoming admissions and manage the admissions/discharge calendar.
Complete social histories upon admission and conduct annual assessments to ensure the emotional needs of each resident are met.
Coordinate care planning meetings and provide follow-up services after discharge to help families access community resources.
Support families of day students in accessing wrap-around services for additional support.
Coordinate short-term stays as needed for families seeking additional services.
Family Liaison and Resource Support
As the Social Worker, serve as the primary liaison between families and Cedarcrest staff, ensuring open and effective communication.
Help families understand the services provided and the community resources available to them.
Encourage family engagement and maintain ongoing communication to ensure families feel supported throughout their child’s experience.
Update and maintain family demographic information in the EMR system, notifying staff of any changes.
Schedule family meetings at convenient times, including evenings or weekends, to ensure accessibility.
Collaborate with the recreation team to organize special events for families.
Develop and distribute quarterly newsletters to keep families informed about Cedarcrest activities and initiatives.
Present resident rights during orientations and mandatory sessions to ensure families are informed.
Lead and coordinate quarterly parent education and advisory council meetings to foster family involvement and provide resources.
Interdisciplinary Collaboration
As a Social Worker, interact with residents and staff to promote the social and emotional well-being of each child.
Advocate for the rights and dignity of residents and their families, ensuring that their voices are heard in care planning and decision-making.
Participate in interdisciplinary care planning meetings, providing valuable insights into the social and emotional needs of residents.
Write quarterly social progress notes according to the established schedule, ensuring accurate documentation of residents’ progress.
QIDP Responsibilities
Demonstrate knowledge of Social Worker roles and responsibilities, as well as federal and state ICF/IDD regulations, in overseeing the QIDP function for all children.
Ensure that residents live in a safe, clean, and supportive environment that fosters dignity and independence.
Collaborate with the multidisciplinary team to monitor and coordinate assessments, evaluations, and care planning meetings.
Oversee individual programs, working closely with department heads (e.g., nursing, therapy, education, and behavior) to ensure interventions are appropriate and effective.
Maintain and audit documentation to ensure compliance with QIDP and ICF/IDD standards.
Serve as an active member of the Ethics and Human Rights Committee, advocating for the rights and dignity of all residents.
Qualifications
MSW preferred. Will consider a Bachelor’s Degree in Social Work (BSW) from an accredited program with five (5) years of experience in social services.
Strong understanding of disabling conditions and their impact on the child and/or family.
Knowledge of community resources and the ability to connect families with appropriate services.
Excellent problem-solving skills and the ability to work collaboratively with families, staff, and external resources.
Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
Strong communication and interpersonal skills to work effectively with residents, families, and staff.
Ability to work flexible hours, including evenings and weekends, as needed.
