Access TLC is seeking a compassionate and skilled Social Worker to join our team in the Multipurpose Senior Services Program (MSSP). MSSP is a state-funded program that helps older adults remain safely in their homes and communities by providing coordinated health and social services. The program aims to help participants remain safely in their homes and communities by providing ongoing care management and linking participants to community-based services and resources.
Key Duties and Responsibilities include but are not limited to:
- Collaborate with an RN and other staff to ensure services meet MSSP and state regulatory requirements.
- Conduct thorough psychosocial assessments to identify participants' needs, strengths, and risk factors.
- Develop and regularly update individualized care plans that reflect participant goals and service priorities.
- Provide ongoing case monitoring and support, including regular phone and in-person check-ins.
- Advocate for participants by facilitating access to community resources and liaising with healthcare providers and support networks.
- Maintain accurate, timely documentation using a health management software platform in accordance with program standards.
- Monthly case management calls to maintain ongoing contact with participants and to monitor for changes in health, mood, social integration, functionality, and overall well-being.
- Complete APS reports and related reports to the state based on mandated reporting criteria and follow through with addressing issues and concerns related to changes in condition due to abuse/neglect.
- Follow through addressing participant concerns related to changes in medical or psychosocial condition.
Position Requirements:
- Bachelors Degree in Social Work, Psychology, Counseling, Gerontology, Rehabilitation, Sociology, or a related field.
- Minimum of one year of related work experience
- Experience with, and understanding of the medical and psychosocial problems of: Adults, functionally impaired children, adults with disabilities and older adults
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
- Strong organizational, communication, and time management skills.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
