High Intensive Care Coordinator HICC (Case Manager)- Children's Services
This position will be based out of our Geneva County Clinic in Geneva, Al
Travel will be required to Dale County as well.
Monday-Friday 8am-5pm.
Duties of this position include:
- The Case Manager in this role will provide case management services to adolescent consumers within the Geneva and Dale County Areas
- Interview and determine needs of adolescents/families
- Assist consumers in accessing community services inside and outside of the SpectraCare System
- Implement service plans
- Service as a liaison between consumers of the program and other community agencies
- Maintain client records using an electronic clinical record system
Requirements:
- Must have a BS degree in Psychology or related mental health field
- Must have a good driving record free from DUI/reckless driving for the past (3) years- will be subject to initial and ongoing MVR checks
- Must successfully complete case management training through Alabama DMH/MR and receive certification of that training upon hire
