IMMEDIATE OPENING The Home Health Intake Coordinator is an integral member of the healthcare team. The Intake Coordinator is responsible for processing all agency home health referrals. The intake Coordinator performs family welcome calls to explain all the services the agency can offer and to keep the patient/family informed to ensure all patients have a smooth transition into home health services.

JOB RESPONSIBILITIES INCLUDE:

Receive referrals from referral sources.

Educate patients on the services we provide.

Send referrals to the non-medical Department for potential services.

Determine what services were approved by the insurance.

Basics knowledge of home care

  • Initiates and maintains a high level of customer service and positive employee relationships.
  • Evaluates and process homecare referrals quickly and accurately.

Will obtain initial payor verification for home care services at the time of intake referral processing.

  • Obtains orders, face-to-face visit notes, and any other necessary documentation to begin home care services and to fulfill payor requirements.
  • Works in collaboration with intake RN/LPN, Home Care Clinical Teams, and various members of the Team to expedite referrals.
  • Responsible for answering intake phone calls: will maintain excellent customer service and appropriately answer questions or route calls to the appropriate responsible party.
  • This position will maintain strong collaboration and teamwork throughout all assignments and interactions with all departments.
  • Ability to function effectively under department-specified timelines and strong ability to effectively manage varying workloads.
  • This position will perform other duties as needed at the Manager or Supervisor’s direction.
  • Knowledge of Medicare regulations and reimbursement.
  • Proficient in Microsoft Office 365,
  • Wellsky Software knowledge preferred and/or healthcare EMR knowledge.
  • Investigative ability, highly organized, self-motivated, takes initiative, excellent written and verbal communication, and critical thinking skills.

Schedule:

  • 8-hour shift
  • Monday to Friday
  • Work Location: In person
