About Job

Qualification Requirements

  • 1 year of social work experience preferably in a health-related facility and a Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; OR, 3 years of social work experience preferably in a health-related facility with a Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
  • Currently licensed as a social worker/counselor in the state of Ohio and is eligible to obtain a Michigan social worker/counselor license.
  • Possesses a thorough knowledge of local and state human service agencies.
  • Has the ability to work independently and within an interdisciplinary team.
  • Possesses strong verbal and written communication skills.
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
  • Understands and is committed to the hospice philosophy.

Essential Job Responsibilities

  • Manages an ongoing caseload of bereaved families/caregivers; performs on-going psychosocial assessments of bereavement needs of caregivers.
  • Develops, implements and updates the bereavement plan of care.
  • Maintains productivity standards as defined by the agency.
  • Provides short-term crisis intervention and individual or family counseling when indicated.
  • Attends and participates in the Interdisciplinary Plan of Care upon request.
  • Documents clinical information according to agency standards.
  • Facilitates bereavement support groups as needed.
  • Provides day-to-day direction and support to assigned bereavement volunteers.
  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
  • Provides education to the community, agencies, organizations and/or groups related to grief and loss.
  • Maintains the standards of Social Work/Counselor in accordance with the principles set forth by Ohio and Michigan State Health Departments, State of Ohio Social Worker and Counselor licensing board, Michigan Board of Counseling, Joint Commission, Midwest Care Alliance, and all agency policies and procedures.
  • Performs other duties as assigned.

This position may have exposure to infectious diseases. This position will require driving to patient homes and facilities in various weather conditions and will be subject to day and/or night-time travel.
