Key Responsibilities
- Coordinate post-acute discharge planning and referrals
- Handle care coordination and DME (Durable Medical Equipment) orders
- Work in an acute hospital setting with an average daily census of 20
- No direct utilization review required
✅ Requirements
- MI RN License – Must be active and in hand
- 1+ year of Case Management experience
- EPIC documentation experience required
- Comfortable with post-acute care referrals and Allscripts
- Experience in ICU, Med-Surg, or ER preferred
- BLS preferred, not required
- Must live more than 50 miles from the facility
- Must meet ProMedica behavioral standards and comply with strict call-off policy
